How do I request a Nutrition Consultation?

 

To request a MyConsult Nutrition Consultation from the Cleveland Clinic you will first need to register to use the site. During the registration process, you will establish a secure user name and password. Once you have established a user name and password, you can access the e-Cleveland Clinic MyConsult Nutrition Consultation service from any computer with Internet access and a browser capable of 128-byte encryption (Internet Explorer 6.0 or higher). If your employer is providing this service, you may contact your human resources department for additional information or instruction.

Upon log in, you will be in a secure Web environment -- only you and The Cleveland Clinic can read any information you send. (See our privacy policy for more information.)

There are seven steps to the consultation process, which will take about 45-60 minutes to complete. It will be helpful to have a copy of the patient's medical information relevant to this nutrition consultation with you as you complete the medical history questions. You will be notified by e-mail when your consultation results are ready. An e-Cleveland Clinic staff member will be available to help you if you have any questions.

Each step of the Request Nutrition Consultation process is outlined in more detail below:

Step 1 - Requester Information
In this section we ask for basic registration information about the requester. This section takes about 5 minutes to complete.

Step 2 - Patient Information
In this section we ask for basic registration information about the patient. You may make a request on behalf of someone else if you have the patient's permission. The patient will need to complete a proxy form and it must be faxed or mailed to us before the nutrition consultation will take place. This section takes about 5 minutes to complete.

Step 3 - Select Condition
In this step, tell us the main reason you are seeking advice from the dietitian. This request will commonly be based on a specific diagnosis or condition (e.g. high cholesterol, high blood pressure, obesity) but can be made for disease prevention reasons as well. This step will take 1-5 minutes to complete.

Step 4 - Payment Process
You may pay for your remote nutrition consultation using your credit card or with a check or money order. If you will be paying with a credit card, your credit card information is required to process the request for a nutrition consultation. If you are receiving this service as a benefit from your employer you will not be required to enter your credit card information, but you MUST request your remote nutrition consultation through your employer's Intranet location in order that you employment and benefits status may be appropriately verified. This step takes about one minute to complete.

Step 5 - Medical History and Nutrition Questionnaire
In this section, we will ask for specific information about your medical and dietary history. Depending on your condition, you may need to have some of your medical information with you to complete the section. For example, if you have high cholesterol, we would like to have your most recent cholesterol values. This information is submitted online and may take up to 30 minutes to complete.

Step 6 - Information Checklists
In this section, we provide lists for you check off to assist in completing your request for a nutrition consultation. This takes 5-7 minutes to read and complete.

Once you have completed all of the material and submitted your request, it may take up to 10-14 business days to complete.

Step 7 - Review and Authorization
This section allows you to review and change any of the information you have provided. If you are charging the service to your credit card, this step also authorizes us to charge your card. Your credit card will NOT be charged until the remote nutrition consultation is posted to the secure Web site. Once you have submitted your request, you may check on the status of your nutrition consultation by logging on to the secure Web site. The opinion status will be displayed on the home page. In addition, we will notify you be e-mail when your consultation is ready.
